Role Purpose

To provide practical, short-term support to people returning home from hospital, helping them remain safe, independent and connected to their local community during their recovery.

Main Responsibilities

  • Carry out welfare check telephone calls where appropriate.
  • Provide a friendly and supportive point of contact for individuals following hospital discharge.
  • Signpost service users to relevant community and voluntary services.
  • Record activities and report any concerns to the Hospital Discharge Support (HDS) Coordinator.
  • Follow all Brentwood CVS policies, including safeguarding, confidentiality and data protection requirements.

Boundaries of the Role

  • Volunteers do not provide personal care or medical support.
  • Any safeguarding or welfare concerns must be reported immediately to the HDS Coordinator or Director
